Can I use my insurance for the visit?

Absolutely! We are credentialed with all major insurance companies and Medicare/Medicaid services. Of course, always check with your insurance to verify your coverage for preventative visits and copays.

The Avery Reset & Reboot is cash/credit card only. We’re looking into HSA coverage.

Where are you located?

The office address is 9537 S. Pulaski, Evergreen Park, IL. 60805.

What Lab services are used?

We are primarily using Labcorp and Quest. In addition, some specialty labs for DUTCH testing and more.

What billing company do you use?

J&J Consulting and billing manage all insurance claims and concerns, please call 888-827-5423 if any questions about your billing. I appreciate your support.

Are you seeing pregnant patients right now?

Unfortunately, at this time Mary Kay isn’t taking any pregnant patients. Feel free to reach out though, I’m happy to refer you to one of my physician or CNM colleagues based on your desires and goals.
